ive had that once or twice... but only when , when some1 quit the game properly but didnt quit just stood there the ghost image appeared... i tried kiling the ghost didnt work... untill i killed the guy that quit that was standing there did the ghost go away... another time it was just there everyone was active dont know how it got there must be a rare glitch
